Where pigment spots come from
Behind every brown spot are pigment cells that produce more melanin in one place than their neighbours. The most common trigger is UV light: years of sun lead to the typical sun or age spots, which are usually sharply defined and sit in places that receive a lot of light — face, backs of the hands, décolleté, forearms. A second group develops after inflammation, such as dark marks after healed acne or after a skin injury. A third group has hormonal causes: melasma appears as extensive, blurred tanning on the forehead, cheeks or upper lip, often during pregnancy or when taking the pill, and is intensified by sun and heat.
This distinction is not academic; it determines the treatment. Sun spots generally respond well to light. With melasma, light can further irritate the pigment cells and worsen the condition — here we proceed very cautiously, work more with peels, skincare and consistent sun protection, and decide individually whether and at what energy a device makes sense. You can read more about the background in our guide Pigment spots: causes and what really helps.
When you should have a spot assessed by a dermatologist first
Not every dark spot is a harmless sun spot. Moles that are new, have changed in shape, colour or size, have irregular borders, itch, bleed or appear multicoloured do not belong under a light device; you should first have them checked dermatologically. We treat exclusively benign spots for which this check has been carried out or where the findings are unambiguous. In case of doubt, we ask you to have the spot checked before we plan an appointment — this is not a formality but part of responsible treatment.
Which procedures we use in Wiesbaden
BBL HERO pigment treatment
Broadband light with a filter that is absorbed precisely by melanin. The spots warm up briefly, darken in the days that follow and flake off finely. For clearly defined sun and age spots on the face, hands, décolleté and arms, this is our procedure of first choice; one to three sessions are usually enough. We explain how the device works on the page BBL HERO in Wiesbaden.
BBL HERO Forever Young
If the spots are accompanied by an overall uneven, reddened or dull complexion, Forever Young treats pigment, redness and skin renewal in one session — as a series of three appointments and thereafter as an annual top-up, which can help prevent new spots.
MOXI laser
The fractional laser renews the uppermost layer of the skin and takes superficially located pigment with it. It is an option when extensive, fine pigmentation coincides with rough texture and large pores, and for darker skin types where light is used with restraint. Read more under MOXI laser in Wiesbaden.
Medical peel
Fruit acid or combined peels loosen the uppermost cell layers and can lighten superficial pigmentation. They are gentler than light and laser, require more sessions and are often the better route for melasma. Details under medical peel.
Skincare and sun protection
Without daily sun protection with SPF 50, every pigment returns. In addition, active ingredients such as vitamin C, azelaic acid or retinoids can slow pigment formation; we will discuss with you which skincare suits your skin.
How the treatment works
Medical assessment
We classify the spots, ask about sun, hormones and medication, and tell you whether you should have a spot checked by a dermatologist first.
Choice of procedure
Localised, widespread, light, laser or peel — we recommend what suits your findings and tell you the price.
Treatment
A session takes 15 to 45 minutes depending on the area. With BBL you feel a brief, warm snap, with MOXI a tingling; cooling and numbing cream are available.
Darkening and flaking
Treated spots turn darker for a few days and then come away. Everything for home is in the Aftercare for BBL and MOXI.
Hands and décolleté: the zones that give away your age
Most people think of the face when it comes to pigment spots, yet the backs of the hands and the décolleté are often more affected. Both zones receive sun for decades but are rarely given sun protection: the hands on the steering wheel, the décolleté in a summer dress. The skin there is thinner and regenerates more slowly than on the face, which is why we work with slightly lower energy and keep an interval of four to six weeks between sessions. For the hands, the BBL HERO pigment treatment costs €240 per session, the Forever Young programme €270; for the décolleté €380. Both zones can easily be combined with the face in a single appointment. A practical tip for the hands: in the first few days after the treatment, wear gloves when washing up and cleaning, and reapply sunscreen after every hand wash, otherwise part of the effect is lost again.
What you can realistically expect
Clearly defined sun spots are usually noticeably paler after a light treatment or no longer conspicuous. Widespread pigmentation needs several sessions and becomes more even, rarely completely uniform. New spots can develop over time, especially with continued sun exposure, which is why an annual top-up is part of the plan for many. Possible side effects are redness, crusts, temporary lightening or darkening of the treated area; we explain the risks to you medically before every treatment.

Frequently asked questions
Do pigment spots disappear after one session?
Individual, clearly defined sun spots are often noticeably paler after just one treatment; they first darken for a few days and then flake off. Extensive or deeper-seated pigmentation usually needs two to three sessions a few weeks apart. Some residue may remain, and new spots are possible with sun exposure — which is why sun protection is part of it.
Can melasma be treated with light?
Only with great caution. Melasma often responds to heat and light with increased pigmentation. We therefore rely more on medical peels, pigment-inhibiting skincare and consistent sun protection, and decide individually whether a low-dose light or laser treatment makes sense. An honest conversation about expectations is particularly important here.
Do I need to have my spots examined beforehand?
For clear sun or age spots that have been unchanged for years, usually not. Lesions that are new, changing, irregularly bordered or multicoloured should be clarified dermatologically before any light treatment. We treat only benign, classified spots and tell you openly if we consider clarification necessary.
How long must I stay out of the sun afterwards?
For several weeks you should avoid direct sun and apply sun protection with SPF 50 every day, even under overcast skies. Otherwise freshly treated skin easily forms new pigment. For the same reason we prefer to treat between October and April, and only skin that is not tanned.
What does it cost to have pigment spots removed in Wiesbaden?
A single spot costs €120 per session for local treatment. Pigment treatment of the whole face with BBL HERO is €440, hands €240, décolleté €380. MOXI starts at €390. All prices include 19 % VAT and are billed according to the German medical fee schedule (GOÄ); with the 3-session package you save 15 %.
What side effects are possible with pigment treatment?
Typical are redness, a feeling of warmth and the intended darkening of the spots followed by fine flaking. Rarely, crusts, small blisters or a temporary lightening or darkening of the area occur, especially on tanned skin. We talk through your personal risks before the treatment in the medical consultation.
As with any medical treatment, side effects and risks cannot be ruled out. We inform you about individual risks, possible alternatives and the course you can expect in a personal medical consultation before every treatment.
